Consent Form for Power2 Leaderlab Programs
Solid Line, LLC and its owners, Julie E. Diamond and Lesli J. Mones (“Solid Line”) provide leadership coaching and training services for individuals and groups of all sizes in various settings, including but not limited to the Power2 Leaderlab. Solid Line will provide its best efforts to assist every participant in reaching his or her personal and professional goals.
Participant requests Solid Line’s services and recognizes and accepts the limitations inherent in coaching sessions and training activities. Participant is responsible for any and all decisions, actions, or inactions that participant may undertake during or as a result of any coaching session or training activity and expressly agrees that Solid Line is not liable or responsible in any manner for harm that may result from such decisions, actions, or inactions.
Participant understands and acknowledges that Solid Line does not guarantee any particular outcome from coaching sessions and training activity and that each participant is solely responsible for his or her own results. Solid Line expressly disclaims any guarantee or warranty of any particular result from its services.
Participant understands and acknowledges that coaching sessions and training activity are not a substitute for medical, psychological, legal, or other professional counseling and that said services are not intended to diagnose or treat any psychological or medical condition.
Participant understands and acknowledges that Solid Line is not liable for any damages or harm, including but not limited to incidental and consequential damages that may occur or result from participant’s involvement in a coaching session or training activity. Participant expressly agrees to indemnify and hold Solid Line harmless for any loss or costs incurred by participant or any person related or associated with participant, as a result of participant’s involvement in a coaching session or training activity.
Solid Line expressly reserves the right to refuse service to anyone for any reason.
BY CHECKING THE BOX BELOW AND ACCEPTING SOLID LINE’S SERVICES, EACH PARTICIPANT EXPRESSLY AGREES TO THE FOLLOWING TERMS AND CONDITIONS.
